Apply for a life certificate

A life certificate is international proof that you are alive. Some authorities, such as insurers, may request this. For example, before they pay out money for pensions or annuities.

You will receive the form from the relevant benefits agency. After you have completed the form, you can have it stamped by the Representation. An appointment is not necessary for this; you can stop by with the completed form and your Dutch passport.
